Job Summary:
We are seeking a compassionate and experienced Hospice Patient Care Manager (RN) to lead our clinical team. In this role, you will oversee patient care services, ensure regulatory compliance, and mentor associates while fostering excellence in end-of-life care. You will serve as a leader who inspires, coordinates care plans, and ensures patients and families receive the highest quality of hospice services.
Responsibilities:
Oversee and direct patient care services to ensure delivery of high-quality hospice care.
Manage scheduling for reassigned, missed, declined, or rescheduled visits using sound clinical judgment.
Triage incoming referrals and medical concerns with professionalism and compassion.
Lead/facilitate Interdisciplinary Group (IDG) meetings and coordinate care plans.
Mentor, train, and provide clinical support to team members to drive compliance and development.
Ensure care delivery meets regulatory requirements (Medicare, Medicaid, JCAHO, ACHC).
Support performance improvement and quality assessment initiatives.
Required Qualifications:
Active RN license in the state of employment.
Graduate of an accredited nursing program.
3+ years of direct patient care experience, with at least 2 years in hospice or home health.
Knowledge of hospice principles, regulations, and compliance standards.
Strong leadership, organizational, and interpersonal skills.
Competency with clinical documentation and technology systems.
Preferred Qualifications:
Bachelor of Science in Nursing (BSN).
Previous supervisory or leadership experience.
Familiarity with EMR systems and telehealth platforms.
CHPN (Certified Hospice and Palliative Nurse) certification.
Schedule and Shift Details:
Full-Time leadership role
Onsite hospice operations
Participation in team meetings and quality initiatives required
