Job Title: Hospice Patient Care Manager (RN)
Job Type: Full-Time, In-Person
Location: Cypress, TX
Schedule: Full-Time, Flexible Hours

Responsibilities:
>Oversee and direct patient care services to ensure high-quality hospice care.
>Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits.
>Triage incoming phone referrals and medical concerns promptly and professionally.
>Lead Interdisciplinary Group (IDG) meetings, present patient updates, and coordinate care plans.
>Mentor, train, and provide clinical support to hospice staff.
>Support performance improvement, quality assessments, and operational efficiency initiatives.

Required Qualifications:

Current RN license in the state of employment.
Graduate of an accredited nursing program.
Minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care.
Knowledge of hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC).
Strong leadership, organizational, and interpersonal skills.
Competency in clinical documentation and technology systems.
Preferred Qualifications:

BSN degree.
Previous leadership or supervisory experience in hospice or home health.
Familiarity with EMR systems and telehealth platforms.
Certification in Hospice and Palliative Nursing (CHPN).
