Responsibilities:
Conduct telephonic and face-to-face assessments, planning, and coordination for members
Manage a caseload of 60–95 members
Perform home visits and nursing facility visits within assigned counties
Develop proactive care plans to address issues and enhance short- and long-term outcomes
Coordinate and implement care plan activities; monitor and document progress
Escalate quality-of-care issues as appropriate
Utilize negotiation and motivational interviewing skills to promote lifestyle/behavioral change
Coach and empower members to participate in healthcare decision-making
Ensure compliance with case management and quality management processes, regulatory and accreditation standards, and company policies
Required Skills & Education:
RN with current unrestricted Texas licensure
3+ years clinical practice experience
1+ year case management experience
Proficient computer and documentation skills
Strong patient communication, care planning, and motivational interviewing skills
