Job Title: LPN/LVN

Full-time, Part-time, or PRN

Responsibilities:
Implement physicians’ prescribed plan of care
Conduct patient assessments and coordinate care
Administer prescribed medication, treatments, and therapies
Provide compliant, timely documentation of care in the system of record
Educate and train family members on health promotion
Accept and write verbal orders from physicians, obtaining co-signature of Case Manager as required
Follow up with physician, client, family members, and Case Manager on patient concerns and condition changes
Perform other duties as assigned

Required Qualifications:
Active, unencumbered LPN or RN license in the state of practice
Current CPR certification from American Heart Association, American Red Cross, or BLS
TB skin test (current within the last 12 months)
Six months of prior hands-on nursing experience (preferred but not required)
Reliable transportation
