Shift: 8-hour Evenings (3:00 PM – 11:30 PM) or 12-hour Evenings (11:00 AM – 11:00 PM)

Job Summary:
NLB Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care team. The LPN will provide compassionate, patient-centered care across skilled nursing and long-term care settings. This role is ideal for nurses with SNF experience who are looking for a short-term contract opportunity.

Key Responsibilities:

Deliver direct nursing care to residents, including those with dementia, chronic diseases, and diabetes.

Administer medications and treatments according to care plans.

Monitor patient condition and report changes to the healthcare team.

Document care accurately in PointClickCare EMR.

Collaborate with interdisciplinary teams to ensure quality patient outcomes.

Provide education and support to residents and their families as needed.

Active NY-LPN License

BLS (AHA) Certification

1–3 years experience in LTC or SNF (SNF experience acceptable)

Permanent residence at least 60 miles away from the facility
