*As a Home Health Nurse at TONE, your primary focus will be on empowering our patients to regain independence and improve their quality of life. Your duties will revolve around:*

-Delivering specialized care tailored to the unique needs of each patient, guiding them toward greater independence.

-Conduct comprehensive initial assessments to create individualized care plans in coordination with physicians and the interdisciplinary team.

-Educate patients and their families on effective care strategies and lifestyle enhancements to promote their well-being.

-Coordinate and administer specialized treatments to enhance patient health and well-being.

-Auto insurance & Auto registration
-RN license through the state of Michigan
-1 year of nursing experience in any health care settings
-Associate degree in nursing at minimum
-Home health experience
-Valid BLS (CPR) Certification
-TB result
