Job Summary:
We are seeking a licensed and dedicated Physical Therapist to join our outpatient rehabilitation team. The ideal candidate will provide evidence-based physical therapy services to patients with orthopedic, musculoskeletal, neurological, and/or post-surgical conditions. This role involves performing comprehensive evaluations, developing treatment plans, and working collaboratively to improve patient outcomes in a dynamic, patient-centered environment.

Key Responsibilities:
Conduct comprehensive physical therapy evaluations for patients referred to outpatient services.

Develop individualized treatment plans based on patient assessment and goals.

Administer therapeutic exercises, manual therapy, modalities, and functional training.

Educate patients and caregivers on treatment strategies, home exercises, and injury prevention.

Document patient care accurately and timely in accordance with company and regulatory standards.

Collaborate with physicians, other therapists, and clinical staff to ensure comprehensive patient care.

Track patient progress and modify treatment plans as needed.

Maintain a clean, safe, and organized treatment environment.

Participate in staff meetings, continuing education, and quality improvement initiatives.
