Summary: TotalMed is currently looking for a Manager of Case Management to fulfill a need for one of our primary client's Yakima location! This role will be provide day-to-day leadership for clinical professionals responsible for cross-continuum care coordination. This role collaborates with directors, physicians, community partners, and internal teams to identify improvement opportunities.
Job Description:
Assisting with staff orientation and onboarding
Overseeing daily/monthly scheduling, ensuring daily staffing is appropriate
Staff development, along with overseeing the talent acquisition and retention process
Lead a team of case managers to ensure effective care coordination across hospital and post-acute settings
Collaborate with hospital leadership, clinicians, and community stakeholders to enhance care delivery
Identify and implement strategies for improving patient outcomes and care transitions
Ensure compliance with regulatory requirements and internal policies
Support staff development, performance improvement, and team engagement

Job Requirement:
Active Washington or Compact RN License
3 years of hospital case management experience
2 years of case management leadership experience
EPIC experience
