We are seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to provide care for hospice patients in Thurston County, WA, and surrounding areas. The person in this role would be expected to provide care in diverse settings, including private homes, assisted living communities, and skilled nursing facilities. This position is available as a full-time or part-time role, offering flexibility to fit your schedule while making a meaningful difference.

Job Summary:
The qualified Hospice Licensed Practical Nurse (LPN) provides compassionate care to hospice patients in their homes or care facilities. Under the direction and supervision of an RN, the LPN supports the hospice plan of care and collaborates with the care team to meet the physical and emotional needs of patients and families.
Primary Responsibilities:
Follow all hospice policies, procedures, and care protocols.
Implement care plans under the direction of the RN and IDG team.
Provide assigned nursing care, treatments, and preventive procedures.
Monitor patient conditions and report changes to the RN and physician.
Educate and support patients and families on care needs and symptom management.
Assist in evaluating care effectiveness and adjusting plans as needed.
Maintain accurate and timely clinical documentation.
Attend staff meetings, IDG conferences, and required training.
Participate in on-call rotations, including nights, weekends, and holidays.
Communicate schedule changes and contribute to interdisciplinary care planning.
Assist with procedures, maintain equipment, and observe proper infection control.
Support patient and family self-care techniques.

Compensation:
$32 - $35 per hour, depending on experience.
All rates are negotiable within this range and will be discussed during the hiring process.
Qualifications:
Education & License: Graduate of a state-approved school of practical or vocational nursing. Current and active LPN license in the state of practice.
Experience: Minimum of one year nursing experience preferred, ideally in hospice, home health, or a related field.
Certifications: BLS required; ACLS preferred (based on care setting).
Technical Proficiency: Proficient with EHR systems and healthcare technologies.
Other Requirements: Ability to exercise sound judgment, initiative, and work independently within the scope of practice, and adapt to diverse social, cultural, and religious backgrounds. Must have reliable transportation, a valid driver’s license, and auto liability insurance.
