Registered Nurse – Home Health - Travel - (HH RN)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2142.63 - $2361.27

Position Title: REGISTERED NURSE I Total number of positions needed: 1 1) How long is the contract assignment? 13 weeks 2) Schedule of assignment hours? M – F, 8am – 5pm 3) Will this candidate be visiting patients in care facilities or only in patient’s homes? Mostly Home and ALFs 4) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Depending on reliability, dependability, and skillset. For now, Contract assignment6) What region are you in? Home health or Hospice? Home Health 7) What’s creating the request? - Growth and turnover 8) To which location will this person report for assignment, full address, Including the OU (Location Code): Tara Turner, Director of Ops. 2623 Centennial Blvd. Suite 201, Tallahassee, FL 32308 Location Code: 0421 Please confirm above location is correct and list any other territories that are to be covered by the candidates: 9) How many miles per week will the consultant travel? 30/40 a day? Weekly? Could vary, as we have rural areas to cover. Could range 30-80/day, up to 400 weekly 10) What are the weekly units of productivity expectations? 25/30? 30 units 11) What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook 12) Will there be any on call? If so, how much? No, unless they are willing. 13) Do you want the candidate to have home health experience? Yes, this is a must. Medicare OASIS documentation experience. HCHB would be a plus.
