Overview
We are seeking a compassionate and dedicated Licensed Practical Nurses (LPN) and Registered Nurses (RN) to join our healthcare team. The ideal candidate will possess strong clinical skills and a commitment to providing high-quality patient care in a dynamic environment. As an LPN or RN, you will play a crucial role in supporting patients and their families, ensuring their comfort and well-being throughout their healthcare journey.

Duties

Assess and monitor patient health, including vital signs and medical history.
Administer medications and treatments as prescribed by physicians.
Collaborate with interdisciplinary teams to develop and implement patient care plans.
Educate patients and their families about health management and disease prevention.
Maintain accurate patient records and documentation of care in compliance with healthcare regulations using MEDITECH Software. .
Assist with laboratory procedures, including phlebotomy when necessary.
Participate in discharge planning to ensure smooth transitions for patients leaving the facility.
Provide hospice care and support for patients with chronic illnesses.
Requirements

Valid LPN license in the state of practice.
Experience in MediTech Software a Plus but willing to train.
Familiarity with discharge planning processes is advantageous.
Proficiency in phlebotomy and laboratory procedures is a plus.
Knowledge of aseptic techniques and infection control practices is essential.
Strong communication skills and the ability to work collaboratively within a healthcare team.
