Job Description
Summary:
We are hiring a Licensed Clinical Social Worker (LCSW) for a 13-week contract at its Medford Medical Center. This is an excellent opportunity for experienced social workers who thrive in fast-paced, acute care environments. The LCSW will play a key role in supporting patients with complex psychosocial needs through risk assessments, mental health interventions, and discharge planning.

Responsibilities:
Conduct comprehensive psychosocial assessments and risk evaluations.

Facilitate mental health placements and referrals.

Provide support for substance use disorder (SUD) cases and social determinants of health (SDOH).

Collaborate with interdisciplinary care teams on discharge and care coordination.

Connect patients and families to appropriate community resources.

Maintain accurate documentation in Epic EMR.

Float to units as needed; may include weekend coverage depending on PTO coverage.

Qualifications
Education:
Master’s Degree in Social Work (MSW) required.

Experience:
Minimum 2 years of experience required.

Open to first-time travelers.

License/Certification:
Required: Oregon State LCSW license

BLS certification (if applicable per hospital policy)

Skills:
Strong clinical judgment and crisis management skills.

Knowledge of mental health laws, discharge planning, and SUD support.

Experience in hospital case management or acute care setting preferred.

Proficient with Epic charting system.

Strong communication, advocacy, and coordination skills.

Oregon State LCSW license

BLS certification
